What Does a C-SNP Cover? 

  • All Original Medicare Benefits: C-SNPs must cover everything that Original Medicare (Parts A & B) covers. 
  • Prescription Drug Coverage: Most C-SNPs include Medicare Part D prescription drug coverage. 
  • Condition-Specific Benefits: Extra benefits and care coordination tailored to your chronic condition (for example, disease management programs, nutrition counseling, or access to specialists). 
  • Care Management: Dedicated care teams to help you manage your condition, medications, and appointments.

Who Can Enroll in a Chronic Medicare Advantage Plan? 

You must: 

  • Be enrolled in Medicare Part A and Part B
